DUBOIS – Late registration for entry into the DuBois Area School District in the fall of 2017 will take place Aug. 2.
All students grades K-12 should register at the Dubois Area Middle School. Personnel will be on duty to assist with registration from 9 a.m. to 12 p.m. and 1 p.m. – 3 p.m.
First-time registrants for kindergarten and first grade must bring their birth certificate and social security number.
All students must bring immunization records and proof of residency. (Examples: deed, lease, current utility bill, current credit card bill, property tax bill, vehicle registration or DOT Identification Card.)
All children who will begin kindergarten this fall must be five years of age by July 1. First grade registrants who have not been in kindergarten should be six (6) by July 1.
Parents and guardians should note that state legislation, Act 26, requires that all students enrolling as transfer students from another district must complete a notarized statement of affirmation that they have “never been previously suspended or expelled from any public or private school of the Commonwealth or any other state for an act or offense involving weapons, alcohol or drugs or for the willful infliction of injury to another person or for any act of violence committed on school property.”
This form must be completed prior to student entry to school. Forms will be available at late registration.
