CLEARFIELD – A DuBois woman accused of forging prescriptions and checks pleaded guilty during sentencing court in Clearfield County.
Holly Wolf, 23, DuBois Manor Motel, Liberty Blvd., DuBois, was sentenced to 30 days to one year in jail and two years consecutive probation for acquiring or obtaining a controlled substance by misrepresentation.
In a second case, she pleaded guilty to forgery and theft from a motor vehicle. For this she was sentenced to two years probation, which will run concurrent with her other case.
Wolf was fined a total of $600 plus costs. She was also ordered to complete drug and alcohol counseling and to not have any narcotics without written approval from the probation office.
The drug charges stem from incidents in DuBois and Sandy Township beginning in December 2010. According to the affidavit of probable cause, on March 7, 2011, a woman reported that Wolf had been filling prescriptions in her name. When the victim checked pharmacies in town, she discovered several transactions at Wal-Mart even though the victim doesn’t fill her prescriptions there. These prescriptions were filled under her name beginning in December 2010, with three prescriptions in February 2011.
During an interview with police, Wolf admitted to calling in a prescription for vicodin and ultram without the victim’s permission. Both are controlled substances used for pain. Wolf said she used a fictitious name to obtain the prescriptions from Wal-Mart. She admitted she is addicted to pain medication.
According to the affidavit in the second case, Wolf removed a wallet and checks from the purse of a bus driver. She then forged two of the checks at Martins and K-Mart.
