CLEARFIELD – Vouchers for the Pennsylvania Senior Farmers Market Nutrition Program (SFMNP) continue to be made available to qualifying senior citizens in Clearfield County.
Vouchers are distributed once a year. If you are 60 years of age and older, live in Clearfield County, and meet the income eligibility requirements ($21,590 for a single person and $29,101 for a married couple), you are eligible for the program.
Identification showing date of birth and county (or town) residency is required when vouchers are picked up. Eligible individuals who cannot pick up their vouchers may name a proxy to pick these up for them by signing an authorized proxy form.
Proxy forms are available at all county Centers for Active Living or on the agency’s Web site (www.ccaaa.net). The form must be signed by the eligible individual prior to picking up the vouchers, and identification for both persons must be submitted by the proxy when applying for vouchers.
Voucher distribution is as follows: Clearfield Center for Active Living (new location is 116 S. Second St.), Kylertown Center for Active Living, Coalport Center for Active Living, Mondays through Fridays 9 a.m. – 2 p.m.; DuBois Senior and Community Center, Mondays and Wednesdays, 9 a.m. – 11 a.m.; Mahaffey Center for Active Living, Mondays through Thursdays from 9:30 a.m. – 11 a.m.; Karthaus Center for Active Living, Mondays, Wednesdays and Fridays from 11 a.m. – 1:30 p.m.; Houtzdale Family Service Center, Mondays, 1 p.m. – 3 p.m.; Clearfield County Area on Aging 103 N. Front St., Mondays through Fridays 8:30 a.m. – 3 p.m. Final distribution of SFMNP vouchers will be September 30 while supplies last.
