PHILIPSBURG – Kindergarten registration for the 2007-08 school year has been scheduled for the Philipsburg-Osceola Area School District.
Children who will be 5 years old on or before Sept. 8 may register for kindergarten class beginning March 20. Registration hours are from 9-11 a.m. and 1-2:30 p.m.
The registration schedule is as follows:
MARCH 20 – NORTH LINCOLN HILL – 342-4906
MARCH 21 – OSCEOLA MILLS ELEMENTARY – 339-6812
MARCH 22 – PHILIPSBURG ELEMENTARY – 342-2870
Registration packets will be available at the principal’s office in each building after Feb. 20. The packets contain the following forms: Pupil’s History, Student Emergency Information, Health History, Dental/Medical Consent Form, Home Language Survey, Permission for Testing Form and Bus Transportation Form.
When bringing a child for registration, have the following with you:
• The completed forms mentioned above
• Birth Certificate
• Immunization record (DPT-4 required –1 after child’s 4th birthday) (MMR-2 required) (Hepatitis B-3 required) (oral polio-3 required ) (Chicken Pox vaccine-required).
