DANVILLE, Pa. – A Geisinger pharmacy program was recently recognized for helping patients with kidney disease better manage their blood pressure.
The Ambulatory Clinical Pharmacy Medication Therapy Disease Management Program (MTDM) earned the Innovative and Collaborative Practice Award from the Pennsylvania Society of Health-System Pharmacists. The award recognizes innovative, collaborative programs that have demonstrated a positive impact on patient care.
The MTDM team was recognized for their implementation of a remote patient monitoring program as a pharmacist-nephrologist collaborative model with Geisinger nephrology and Alexander Chang, M.D., M.S., co-director of Geisinger’s Center for Kidney Health Research. These efforts were aimed at improving blood pressure control for patients with kidney disease. The program has decreased blood pressure values and reduced hospitalization for affected patients. It was implemented in 2022 across the Geisinger service area and provided more opportunities for health-system-based pharmacists to have a direct impact on the health and well-being of patients in central and northeast Pennsylvania.
“Programs like this pave the way for new, innovative models that provide patients with access to the experts they need to manage their complex conditions,” said Daniel Longyhore, system director Knowledge Management, System Pharmacy Support Services, Geisinger. “I applaud the team for their collaboration on this.”
This is the MTDM program’s second Innovative and Collaborative Practice award.
