Acceptable proof of identification includes a driver’s license, state ID card, passport, credit card with photograph or school ID. Acceptable proof of address would be any mail that is not of a personal nature.
*** Applicants under 18: without photo identification, they must be accompanied by a parent or legal guardian who has a photo ID and proof of address. The application card must be signed by a parent of legal guardian.
